Child Care Centre Registration

To register your child in a PLASP Child Care Centre, please visit the Program Director at the location of your choosing. Please be aware that there are waiting lists for care in our child care centres and you may not receive care immediately. As soon as possible, please contact the location you wish to register for to ensure space availability.

To ensure your child's safety at our centres, we require that all fields be complete on the registration and medical forms.
When visiting the centre to register, please be sure to bring the following information:

• Address – including postal code
  
• Secondary Address (if different from above)
  
• Employer Information for both parents – name, address (include postal code)
  
• Telephone Numbers – home, work and cell/pager
  
• Emergency Contacts – 2 local people who are aware of our program
  
• Void Cheque
  
• Cheque for registration fee
  
• 2 post dated cheques for first 2 payments (full time)
• Doctors Information - name, address and telephone number (include postal code)
  
• Immunization Record
  
• Allergy Information – Type of Allergy, high or low risk and reaction
  
• Name of any medication taken on a regular or emergency basis
  
• Name of any special medical conditions or special needs
  
• List of any special requirements regarding diet, rest, exercise.
