To register with PLASP, please do the following –

• Download the Registration Form
• Complete all the red boxes on both the Registration and Medical Information Forms
• Once completed, print three copies of each form and sign all the forms
• Bring all three copies of both forms to the PLASP location you wish your child to attend and submit them to the PLASP staff at that school. Remember to bring any “Additional Documents” with you. See below for “Additional Documents”.
• DO NOT mail, email or fax any forms

Information Checklist

To register your child with PLASP and to ensure your child's safety in our program, we require that
all fields be complete on the Registration and Medical Information forms.
You will need the following information, to complete the forms

• Family Address – including postal code
• Secondary Address (if different from above)
• Employer Information for both parents including name, address and include postal code
• Telephone Numbers – home, work and cell/pager
• Emergency Contacts – 2 different individuals who would be available to pick up your child in case of an emergency. The two individuals must have different phone numbers and are aware of this responsibility. Please note your registration will not be processed until you have included this information.
• Doctors Information - name, address and telephone number (include postal code). If you do not have a family doctor, write in the telephone number and address of the medical clinic that you use.
• Allergy Information – Type of Allergy, high or low risk and reaction
• Medication- Name of any medication taken on a regular or emergency basis
• Medical Condition - Name of any special medical conditions or special needs
• Special requirements - List of any special requirements regarding diet, rest, exercise.

Additional Documents Required

To process your family’s registration, please include the following with your forms

• Void Cheque from your account for pre-authorized payments
• Cheque for registration fee (not required if registering prior to June 1st for the following school year)
• If registering for a part-time package, include a cheque for the total amount.
