School Age Program Registration

To register your child in a PLASP program, please visit the Program Director for the PLASP program at your school (between September and June) and complete a registration package.

To ensure your child's safety in our program, we require that all fields be complete on the Registration and Medical Information forms.

When visiting the program to register, please be sure to bring the following information:

• Address – including postal code
  
• Secondary Address (if different from above)
  
• Employer Information for both parents – name, address (include postal code)
  
• Telephone Numbers – home, work and cell/pager
  
• Emergency Contacts – 2 other individuals in the case that parents can't be reached
  
• Cheque for registration fee (not required if registering prior to June 1st for the following school year)
• Void Cheque from your account for pre-authorized payments
  
• Two Post-dated cheques for the first 2 payments (during the school year)
  
li>Doctors Information - name, address and telephone number (include postal code)

• Immunization Record (for children under the age of 6)
  
• Allergy Information – Type of Allergy, high or low risk and reaction
  
• Name of any medication taken on a regular or emergency basis
  
• Name of any special medical conditions or special needs
  
• List of any special requirements regarding diet, rest, exercise.
